Learning Disabilities Supported Living Service is a domiciliary care service

The Bradford Services are registered with CQC as Domiciliary Care Services.

Services provided in the Bradford area are:

Supported Living Service

This service supports adults with learning disabilities to live in their own home.

Floating Outreach Service

This service supports adults with learning disabilities, head injuries and/or mental health issues. The hours of support provided ranges from a small number of hours per day or per week through to 24/7, depending on the individuals assessed needs.

The service works achieving St Anne’s ‘Core Principles’.

We promote:

  • A welcoming and friendly environment for everyone.
  • Both clients and staff to be Dignity Champions.
  • The rights, choices, independence and inclusion of individuals.
  • A person centred approach.
  • Inclusion of the client, their family and other relevant professionals as appropriate with the development and improvement of services.
  • Inclusion of the client, their family and other relevant professionals and writing and evaluating of support plans and risk assessments, goal plans and health action plans.
  • Involvement of the advocacy group meetings for client to offer suggestions and feedback regarding the service provided and received.

We will support you to:

  • Enhance your quality of life.
  • Voice your views/feedback of the service received.
  • Reach your goals and potential.
  • Do your shopping, banking, finances,cleaning, cooking, baking, and meet your health needs.
  • To attend appointments, for example at the doctors, hospital or for your hair doing.
  • Access the community.
  • Write and evaluate your support plan, positive risk assessment, health action plan and goal plan.

Detailed below are just some of the social activities which are arranged at the request of clients to support their hobbies and interests:

  • A ten pin bowling league
  • An allotment project linking with Bradford College
  • Camping Trips
  • Arts and craft sessions
  • Themed evenings
  • Horse riding sessions
  • Line dancing classes
