We support people to live a full and active life by enabling and empowering them to live independently as part of the community, we currently offer support to:
Who should be referred to this service?
Any person with a learning disability, a learning disability with a mental health issue, a person who is on the Autistic spectrum or a person with a head injury.
The person being referred to the service needs to have an identified support need and be 16 or over.
Who can refer?
Referrals will be accepted from anybody and there are no automatic exclusions from our service if you meet the Eligibility Criteria You or someone else must be able to pay for the support service. We can support you to access funding for your support if needed.
