We provide support to adults with learning disabilities who live within Northallerton and surrounding districts. The service is registered with Care Quality Commission as a Domiciliary Care Service

Our aim is to provide support to individuals who require help to maintain a good quality of life and to live as independently as possible.

We adopt a person centred approach and pride ourselves on a multi agency working.

Support packages are devised by the client, Care Co-ordinator and the people who are important to them and who know them best. Support will be reviewed on a regular basis to ensure that is still relevant and is meeting the person's needs. The Supported Living team will provide advice and practical support around budgeting, daily living skills, health issues, social interactions and any other areas identified by the people we support that will promote their independence, opportunity and inclusion.

Domiciliary Care – What does this mean?

The service is registered with Care Quality Commission as a Domiciliary Care Service. This means we will be inspected on a regular basis, to ensure a high level of support is provided.

Who can refer?

We accept referrals from statutory or voluntary agencies, funding should be arranged through North Yorkshire county Council or a Care Co-ordinator.

We will meet with client, referring agency and funders to carry out an assessment of need and any risks identified. This will enable us to decide the appropriateness of the service and agree support needs and funding. A decision will be made by the referral panel and all applicants and referrers (where permission is given) will be informed of the outcome in writing. There is a right of appeal regarding any decision and this will be clearly explained at the time of referral.
