Floating Support Service
Aims and Objectives of the Service
St Anne’s Community Support Service aims to enable people with mental health issues, including those who are in receipt of an Individual Budget or Direct Payments, to live independently. The team offers regular planned, practical, emotional and social support to the individual and their carer, complementing the role of the Community Mental Health Team and other support organisations in Sheffield.
What does the Service offer?
The service can offer clients support in the following areas:
- • life skills
• accommodation problems
• budgeting/managing personal finances
• daytime and social activities
• employment, training and volunteering opportunities
• individual rights
• social networks/relationships
• mental and physical health issues
The Community Support Service aims to be flexible to meet the needs of clients. If the support required is not mentioned above please contact the service manager to discuss your needs.
Who should be referred to this service?
The service offers support to people 18 or over who are experiencing mental health issues and who have a personal budget. For more information about the eligibility criteria please contact the service manager.
Who can refer?
Referrals are usually made by adult mental health professionals however self referrals can be accepted if the individual is in receipt of a personal budget.
