Positive Pathways supports people with mental health issues in Leeds, working on housing and other needs to help with recovery. The service provides both emotional and practical support guided by support planning that involves both the individual and their Housing Support Worker. The service is led by Community Links in partnership with Touchstone, Leeds Mind, Leeds Irish Health and Homes, St Anne’s Community Services and Leeds Federated Housing Association.
Aims and Objectives of Service
The purpose of our service is to deliver good quality housing related and wider support to individuals in housing need who also have issues connected to their mental health. This is in order to enable recovery, improve health and well-being and achieve independent living.
What does the Service offer?
The St Anne’s Positive Pathway service provides support specifically for people who experience coexisting mental health and substance use difficulties; this is often called dual diagnosis. The service adopts an integrated model of care supporting people to address both their substance use and mental health difficulties. In addition, the service works collaboratively with other drug, alcohol and mental health services throughout the city to ensure people get appropriate access to the services they require to meet their dual needs.
Support workers have a flexible and person centred approach and will work with clients to design a support plan around their needs. We use an assessment tool called Outcomes Star which will show a clients journey of change through the service, and this will be reviewed regularly. There are also opportunities to access specialist employment support, volunteering, befriending and peer support services.
All support workers receive specific training to enable them to support people with dual diagnosis. The service is an active member of the Leeds Dual Diagnosis Network, a multi-agency network developed to ensure that services that come into contact with this client group are readily able to assess, engage and to co-ordinate care effectively. Who should be referred to this service?
People must meet the below criteria:
- An individual/family who lives in or wants to live in Leeds and have problems with their housing
- Have needs around their mental health
