St. Anne’s has been providing supported living services since 1995. The service has been developed based on 5 principles :
- Housing and support are separate. Clients can change their care provider, or alter their care package without moving home.
- Person centred approaches ensure that planning focuses on individuals, with the individual being at the heart of the planning process.
- Full user choice and control means that the individual gets the service they need and want, rather than having to fit into existing structures.
- Nobody is “too disabled” to benefit from supported living. The environment and the support package is tailored to individual needs.
- Focusing on relationships means building a network of support for individuals to reduce dependence and increase participation in the community.
Where personal care is provided in addition to support services, the supported living service will be registered with the Care Quality Commission as a domiciliary care service.
The supported living service provided by St. Anne’s turns the 5 principles into reality by
maximising service users' independence. The service people receive is based on need. We can provide support for a few hours a week to or for 24 hours a day, 7 days a week.
The needs of the individuals we work with vary greatly. We recognise that some people will
need life long support, whilst for others their support needs may be relatively short term.
Referrers or applicants to these services need to complete an Application Form and a Third Party Risk assessment. When the forms have been completed please send them to:
