About the Service

Leeds Supported Housing Service provides a wide range of housing across Leeds, from self contained flats to tenancies in shared houses. The service is intended for single people from 18 years old upwards who have medium to long term housing supported needs which can be met by the delivery of up to two hours visiting support per week. We are able to work alongside other agencies for those people who require more intensive support.

Aims and Objectives of Service

We aim to provide quality housing with tenancy related support to enable people to maintain independence in the community. We aim to provide support that is flexible to individuals, their objectives and their changing needs. We will help tenants with a whole range of housing related support to successfully sustain a tenancy.

Using an outcome focused support planning tool, the actual support package delivered is negotiated with the client to enable them to maintain the tenancy. The service is person – centred and works towards the growth of confidence, recovery, independence and empowerment of the individual.

The support that we are able to provide includes :

  • Managing a tenancy
  • Managing finances and budgets
  • Claiming benefits
  • Settling into and becoming involved in the community
  • Accessing cultural and faith needs
  • Liaising with other agencies
  • Promoting physical, emotional and mental well-being
  • Developing self care skills
  • Supporting education, employment and leisure aspirations
  • Encouraging client involvement

We take a holistic approach by addressing all aspect’s of a client’s life – ensuring as far as possible, that all support needs are addressed; whether directly by directly by the service, externally by other agencies or by the client themselves.

The service seeks to offer equal access and opportunity for all individuals and sectors of the community.

How do we support?

The service is intended for single people from 18 upwards who have a medium to long term housing suppported needs which can be met by the delivery of up to two hours support per week.

They also may concurrent or previous experience of:

  • Mental health problems
  • Learning disabilities
  • Alcohol and/or substance use problems
  • Episodes of homelessness
  • Vulnerability related to housing.
  • Significant periods in hospital, registered care or Prison
  • Offending Behaviour

Referrals to the service are accepted from health and social care professionals; the team at Leeds Supported Housing Service are always pleased to have the opportunity to discuss how their service needs

Coronavirus (Covid 19) Statement

Due to the exceptional circumstances due to the ongoing Coronavirus (also known as Covid 19) and the Government rules about self-isolating it is very important to reduce the risk of cross infection to all our Clients and staff teams. Our teams are working hard to support all our clients and are committed to keeping everyone as safe as possible.

This means that, in most circumstances, and in line with government information regarding self-isolation and staying at home there should be no visitors to any of our services.

We know, however, that this will be very distressing for some of our Clients.

Therefore, we are saying there must be no visitors to services unless there are exceptional circumstances and the visit is essential.

Before a visit can take place, this must be risk assessed by the manager considering government advice and the individual circumstances. Before setting off on any visit to your relative/friend please get in contact with St Anne’s staff to discuss your visit.

Our services are using all kinds of technology, including social media to make sure that the people we support can stay in touch with those important to them

St Anne’s has developed a visitors protocol to enable visits to begin again in North Yorkshire, East Yorkshire, Durham, Middlesbrough and Leeds care homes.

In line with Government & local Council Guidelines each Client may have one family member or other person who is close to them visit on a regular basis.

Decisions where visitors can go are being based on Individual Local Authority advice following Government guidelines.
