Denise Jabbi the FLM at Daleholme nominated her team for a Top Team Award.

Pictured are the team at Daleholme with their top team award.
In her nomination Denise told me about her team and how they had all pulled together and dealt with ongoing physical and mental health issues with two clients. She described how one client who was in respite and had a lot of negative input from a family member but that staff always dealt with the relative professionally with the full support of the clients care assessor. She explained that one client whose care needs were complex had needs that changed from day to day.
Along with caring and supporting the 3 other clients that live at Daleholme the team all worked professionally and positively with all the external agencies to make sure the clients received the full support and care required for their individual needs in a very person centred way. Care plans were changed regularly and staff followed these making sure the clients care was very consistent. Staff also spoke up for the clients that cannot speak up for themselves and the service has had positive feedback from Families over the care given. Denise explained that the team have undertaken specific training in relation to the clients changing needs to enable them to support the clients fully and understand the care to be given and made sure the clients have been at the centre of everything making sure that even with staffing vacancies the clients have had full support when needed to attend appointments and planned activities.
In support of her nomination Denise went on to say that the staff have covered shifts sometimes at short notice, covered as extra and stayed on when needed to enable her not use agency and that without the team pulling together the service would not have carried on as positively and effectively as it is. She finally described how a new young adult had recently moved into Daleholme long term and the staff and family are supporting him with the transition from leaving home into care and that the move is going well, and regular structured activities have been sourced and attended and that there had been a lot of interest in the void room and a lot of positive comments from numerous care assessors in regards to the staff team and their professionalism.
Denise described how proud of the staff and the service they provide at Daleholme. I felt there were a very worthy winner and it was a great example of everyone working together during a difficult time. I was pleased to surprise them at a team meeting recently and present them with a well-deserved Top Team Award!
